Nope, he was on a time lord space station.
Just after Jamie and Zoe were sent back to their worlds, the Time Lords passed sentence on him. The sentence was to be exiled on Earth and have a new appearance. They showed him several to choose from, but he couldn't choose, so they said they'd pick for him.
Next thing you know it shows his face being blurred, with other blurred images floating around it. While he protests that they can't do this to him, his face sorta morphs into Pertwee's, but still somewhat blurred. Then the final credits come on.
Since it shows him falling out of the TARDIS already changed in Spearhead from Space, they likely dumped him into the TARDIS after the regeneration and then set the controls for Earth.

Just finished watching it, as I forgot I had downloaded it earlier this afternoon. Not a bad start for Mr Smith as the good Doctor. I liked it. Looking forward to the rest of the season.

I'm confused ... I have watched the first 2 minutes of 11'th hour ...
When did the Doctor regenerate? Does it flash back in this episode as to how and when this happened?

Back around Christmas, New Years. The End Of Time part 1 and 2 he regenerated. No Flashback so you will have to watch those two episodes for the regeneration. I would guess BBC America will show them this week or next before they air the The Eleventh Hour. They were showing the The Waters of Mars last night on BBCA so The End of Time should be next.
