Dr Who - Speculations on the cause of the crack

My guess is the Tardis going up in flames when he regenerated last year, evidence by what he pulled out of the crack.
Still curious why that happened though, when about half of his regenerations were in the Tardis and this wasn't the first one caused by radiation.
Any other guesses?

That piece looked to be from the new TARDIS not the old one.

I am not very imaginative when it comes to these things. It usually hits me like a ton of bricks when it is revealed. I am like *that is so obvious why didn't I think of that".
My thoughts were perhaps it had to do with Time Lords trying to break the time lock or perhaps the Cybermen breaking out of the void.
