$75 VISA Gift Card. Customer Subscribes to a minimum basic programming package of Americas Top 120 or above, signed a 24 Month commitment with DISH Network and signed up for Credit Card Auto -pay on the day the initial order was placed. (DHA 24 Plans Only)

$125 VISA Gift Card. Customer Subscribes to Minimum basic programming of Americas Top 120 with a Digital Video Recorder, signed a 24 Month commitment with DISH Network and signed up for Credit Card Auto-pay on the day the initial order was placed.(DHA 24 Plans Only)

To be eligible for this rebate offer you must have signed up for New DISH Network service through DISHSTORE between 5/30/2011 and 1/31/2012

If you use a Club Dish referral certificate when you placed your order, your rebate amount will be reduced by $50. NO EXCEPTIONS

Gift card requests are automatically processed when you order your DISH Network service through the Dishstore by calling 1-800-807-0837. It is not necessary to fill out this form to receive your gift card, however filling out this redemption form will speed up the redemption process.

After your service is installed you will receive a certificate in the mail to redeem your gift card. Gift offer requires mail-in redemption within 60 days of receiving certificate. Customer must pay for shipping/handling, and return a copy of their 1st month Dish bill. Allow 2-4 weeks delivery. One gift per activation. Dishstore reserves the right to substitute the gift.

Gift Card redemption requests will not be processed if your account is disconnected, or if you ordered your service through another DISH Network retailer.
