Just look on the Solid Signal website. Took about 15 seconds to google them and look for myself. Might be able to pick one up cheaper on eBay.

If you are getting a 100% signal reading on channels it could be the signal is too strong. This is not an uncommon issue even before everything going digital. The Dish rtuner may be more sensitive to strong signals.

The last station that was blanked out was 26-01 which was a Scripts station. It has back for a number of months. We have guide data for all of the Big 4 networks. So, it appears that we can't just go by that data list.

IMHO, contact your homeowners insurance company and have them cover it. I doubt Dish will cover it. Your only chance may be if you can provide proof that the installer insisted it be installed that way and you can prove that the install was defective.
