622 error 004

I keep getting a error 004 " a problem detected with my switch" Then it goes to test the switch and never finishes. stays on 0/5 the funny thing is I can still hear the audio from the program in the background. Dish came out and set it was the box, but the said I have to wait for the next software update. I have had this thing a lil over a month now. It doesnt record all of the programs also. tried recording 24 season finale and it got 32 min of 2 hrs, pink pntrher fo rthe son 12 out of 32 min. the shows it does playback a screen pops up and says part of the recording lost due to signal loss however the video is fine behind the message then it goes away and the audio makes a loud screeching noise. It doesnt seem as if anyone else has these issues either, I think this thing is a lemon.

I have a D1000 not switches straight from the lnb to the receiver.
