DVR Cost Question?

I have two HD Receivers and am thinking about a HD-DVR, to replace one of them. I am trying to figure out the cost of the DVR.

1. Do you pay the $4.99 for the second receiver and $5.99 for the DVR? Or just one charge?

2. If you take the Premier Package, is the DVR fee waived?

I see different references on the D* web site, but want to make sure I understand it before I make a purchase.

You will pay $4.99 for each additional receiver and you will pay 1 fee for all Tivo's on your account.

Tivo fee is included in the Premier pack.
