If you buy the MLB package does that include the region you are in, such Fox Midwest for my Cardinal games or is it just the out of market channels.

We do have EI. Usually we get the SD feeds provided by MLB EI for games on channels that Dish doesn't carry. For example, if the Mets were playing the Blue Jays, we'd see the game in SD from whatever feed was provided by the MLB.It’s the in market stuff they would black out.
So you either need to spoof your address move.
Speaking of moving. Does Dish have MLB extra innings and if so would you get all the out or market games ?
