How do I RA my old reciever

I just got a replacement for a defective reciever. Now I need to send back the old one but I did not get a ups label. I did find the return authorization number but the is no label with Dish's return address. Am I missing one or not reading the directions right. THANX

If you have the Protection Plan call them and get a label sent. If not, you'll have to send it yourself. Use FedEx or UPS and make sure you specify the value and track the shipment. I'd print out the delivery information including who signed it whether it's at your expense or with their label.
