New Dual Tuner OTA Adapter

Neither of those will work because it's similar to video cards using ATI/AMD or nVidia for example. Both ATI/AMD have their own video cards that are made by them and sold as retail products with a display box that comes with a warranty and then you have the cards that are made by them that only come in a anti-static bag without the box which are considered OEM versions and do not come with a warranty other than the merchant you bought it from. No matter what, ATI/AMD will not honor the warranty on the OEM model. Same with nVidia, nVidia does not make their own graphic cards as even the chips just like ATI/AMD are made by Taiwan Semiconductor aka TSMC, all nVidia does is provide a reference diagram and other companies makes boards based on it and brand it as their own with the model called nVidia and they are the ones who provide the warranty. Even for hard drives and talking about bare internal drives, there is the retail version which comes with a manufacturer version and then there is the OEM version like the ones inside a Dell computer that does not have a manufacturers warranty but only one from Dell or whoever the computer came with. You can buy the bare drive from stores too which only has a warranty provided by the stores. When you call the manufacturer of the hard drive in question even though it has the same exact make/model number, there is no warranty and they have no liabilities at all since it's the vendor who provides the warranty and not them which is also another reason they get the parts cheaper as it costs less.

The dual tuner is not Dish branded. As a matter of fact it has no branding whatsoever.
All of the 'marketing' pictures showed it with the Dish logo. There was a thread where members were posting pictures of theirs but I can't find it now. The listing at Dish Depot shows the logo too.

Obviously I don't or I would know if the shipped models have a logo.

This is really odd.... It's another possible explanation why Dish doesn't sell them though. But JSheridan has said his Dish equipment supplier has them and is shipping them to Dish installers, retailers, etc.

Folks it is a $60 electronic item with no moving parts. Chances that is will have a manufacturing defect are very low. If you have a need for it buy it. Worst case scenario if you happen to be the 1 in 500 or more that it flakes on after 6 months you are out $60 buy another and move on life is to short.

Seems to me that quite a few people here are reporting issues.
