Received mail from Dish

I received some mail from Dish last week (I don't remember everything it covered). But I remember it listed some dates when they would be offering the different movie channels on freeview. My wife threw the mail away and I don't remember what it said. Can someone post those dates? I assume everyone got the mail.
 
My locals are not separated as a line item and I did not get a letter for free HBO, but then I only get the Welcome pack. So, they would lose money on me.

I have the option of a month of Epix channels, a month of Showtime channels, a month of Outdoor pack, a month of Dish Movie pack (of which Epix is included...), or 1 PPV.
