I can't imagine Dish letting a customer go because they won't offer them the HD for free when the time comes. I believe the only reason they changed the Free HD promotion was for marketing reasons anyway. Like I said, I wouldn't worry about the Free HD at all.

Why is there a fee to receive HD? Does it cost more to broadcast it?

Ask the channel owners. ESPN/Disney HD channels were off Dish for a very long time because of that.

Depends on the network. That's why Dish didn't have ESPNU and other Disney channels in HD for a while. Disney charges providers for the SD version and the HD versions of their channels. Most of us here like to call that double dipping.
