I doubt this would have happened if the dish had not been bolted to the motor shaft.

It also would not have happened if the dish could not hit an obstruction,

That would fall under the category of "installer error"....never mount a dish where it can hit an obstruction,

also sounds like the motor might have been overloaded with a heavy dish,
