Use a small (1/8" - 3/16") long pilot bit to drill from the inside out. If you measured things close, all you will hit outside is air. Then with a holesaw sized for the PVC pipe LB you are using drill from the outside in. you will have a very clean hole easy to seal and a close fit to your PVC...

Get a long extension cord and take the box to the dish with a small tv, or get a pair of diplexers and push the receiver output back out to the dish.

I have an AZBOX, and despite it's quirks for the last 3 years it has been a good receiver. Compared to what you can get for 1/2 the price these days....unless you're a real PBS fan or have to have all the bells and whistles and are willing to put up with the all hassles of getting it set up...
