On the Dish'n It Up! web page, it says the following:

"Shipped HD DVRs subject to certain restrictions"

Does anyone know what the restrictions are?

I assume you mean that I have to subscribe to an HD package and extend my contract. Anything else?

Ha!. No. I am looking at $75 for a 612, $100 for a 622, or $125 for a 722 according to the site. Are those prices pretty standard?

Those prices are considering that you are a plan A Dish account. When you request the DIU promotion, that will be finalized.

I don't suppose it is too much to ask what a Plan A account is, is it?

Plan A is the highest credit rating that an account can earn. The criteria is determined by Dish. That info is unknown. To find out where you qualify, call 800 333 3474 and ask for technical help. A live CSR will identify your account and inform you of your plan level. Your local dealer, should you have one, can go on line and the system will tell him your plan level. You will need to supply your account # and a CC#. You can only do this if this local dealer was your origional selling dealer. The term is ROR.
