I had to figure this out for myself, so I wanted to share for others. The dish CSR's are worthless.
On June 3rd, I went into my account details. I was Turbo HD Bronze, and I switched to America Top 120. I then signed up for paperless and auto-pay. But I was defaulted to the NONE OF THE ABOVE options. When I would select HD for Life, it would just send me to the info page and tell me about the two options to get it. But you couldn't select anything.
I spent hours on the phone and finally tried to do something on my own and it worked. I said to heck with it and updated my account to select the $10 HD 120. I update my account with this, then I went back and selected the HD for Life free option. THEN, finally it gave me the option to add a 2 year committment or pay a one time $99 fee. I went with the comittment, as Dish is MUCH cheaper than my local cable company.
