Thought I might give another go at an update, with slightly reshuffled categories.

You might consider this less compete, as I removed some channels that make less difference, such as the FiOS controlled .TV channels, and west coast versions of premium channels. Thanks to Adam, who started this list. Feel free to correct.

Many of the channels have no extra charges for the HD version via the SD version. The movie channels are an example of it. Most of the HD carriage is a negotiated item with programming providers.

Launching satellites enables more efficient use of existing space, but not more space. At least not until a revers DBS bird comes out, but that will require LNB swaps and potentially new dishes.

And as was said, HD usually cost no extra. Programmers do try to go after an increase, but it is for both the SD and HD versions.

I do not see Dish network ever using reverse DBS for home programming. Reverse DBS has big rain fade issues compared to Ku DBS. Dish has never used their Ka frequencies for home use which have far less rain fade issues than reverse DBS would have. Dish will probably use for back hauls and other commercial purposes.

Reverse DBS even has "rain echo" problems if you are near an uplink center when it rains the powerful transmission from the ground antenna to the satellite reflects back off the raindrops to the area around the antenna and can block the weak satellite signal.

Not to accuse Dish of "warehousing" spectrum, but they have had a few Ka licenses lost because of lack of progress in building a satellite, and have had to defend lack of use of others. I suspect something similar might happen to the reverse DBS slots they have applied for.
 
More channels coming, time for an update. There are only 12 standard channels confirmed to be available in HD that Dish does not have (2 likely to be crossed off the list in a couple weeks). The big ones still missing remain ESPNU (awaiting litigation over best customer pricing) and Rainbow channels AMC, IFC, and WE (awaiting litigation over Voom).

Of course, there are 14 more that Dish has in SD that are merely awaiting and HD launch. And there are 14 premiums Dish has in SD and does not have in HD. So, aside from RSNs, there are 40 channels to go for Dish to practically have the whole lineup in HD.

The inclusion of nonexistent HD channels in the list is confusing. In particular, what's up with all of the listed Encore HD channels? AFAIK, none of them exist.

IMO, all channels that aren't broadcast in HD shouldn't be a part of this list -- a list that purports to be a run-down of DISH's non-carried HD channels. I would suggest that a separate reference list of channels not available in HD be established instead.

Limited bandwidth. Dish uses 2 transponders for HD RSNs. Thus, they can only hold 14-16 of the 26 of them at once. The only way I think we will see 24/7 HD RSNs is either after the Western Arc MPEG4 transition, or if they go on spotbeams.
