Rebate form?

Hi:

I've been searching the E* site as well as boards and can't find this rebate form? I did upgrade at the beginning of this month and now I need to get the paperwork in. The CR told me they'd send me a form but now I'm getting worried maybe they won't. Can someone please link me the rebate site?

Nick, it is on the Dish website. You will have to log into the Dish site with your account, and then it is on one of the drop down menu selections, I think under Programming upgrade. (gone to sleep and ate dinner since then !)
I just found it yesteday and sent it off to Dish. While you are on the dish site, get a copy of your last months invoice showing that you have indeed upgraded to HD programming. You will have to send that in with your filled out rebate form.
