What channel is CBS-HD?

I recently got a waiver from local CBS affiliate and subscribed to locals. I faxed in the waiver. I am still not getting CBS-HD. What channel it is on? What time does it approximately take after you fax-in the waiver?

CBS-HD East is ch 9483 on 61.5
CBS-HD West is ch 9484 on 148

Normally, Dish sends the waiver request to the stations and then it takes six weeks. Several years ago, when I got my waiver, Dish would not accept a waiver that you had gotten. I sent them a waiver and then they sent a request to the station themselves. Of course, I did not get the CBS HD until they got their approval back.
