My owned 622 DVR recently started having problems. It would
flash Error 122 (Internal communications failure) and Error 005
(Smart card not authorized) every few minutes. I chatted with
Dish and Tier 2 Tech Support said the receiver needed to be
replaced. Since I owned my equipment, they charged me $69.63 and
sent me a replacement receiver.

I installed the replacement 622 DVR and immediately ran into more
problems. The HDMI output did not work so I connected it using
component cables. Then a second problem occurred. I kept
getting Error 004 (Channel Signal Loss) when switching to HD
channels on 129. I had to channel up and down many times to get
the HD channels to come in. I chatted with Dish again and Tier 2
Tech Support agreed to send another replacement 622 DVR at no
charge.

I installed the second replacement 622 DVR and thankfully the
HDMI worked fine. Unfortunately I was still getting Error 004.
I chatted with Dish again. Tier 2 Tech Support said a tech would
have to come out. They said it would cost $95. I asked what the
tech would do and they said he would replace the lnbfs. I said I
could buy new ones and replace them myself for less.

I'm still not certain if Error 004 is caused by a defective
receiver or a defective lnbf. If the receiver is defective, then
Dish should replace it again at no cost. If the lnbfs are bad, I
have no problem replacing them. But I don't want to waste time
and money replacing them if they aren't causing the problem.

Can a defective receiver cause Error 004? If it is caused by a
bad lnbf, would it be the DPP Twin or the 129 dual?

If your LNBs haven't been replaced since that dish was put up, they're probably at least 5 years old since the 1000 dish hasn't been installed in a while. Hate to say it but, call in. Complain just enough to get to loyalty, if you're a good customer they may waive for the tech and he can put up a 1k2 LNB for you

Yes, my Dish 1000 is over 5 years old. I bought it when they first came out. Lnbs are original.

I'm experiencing the same 004 error, but it's on my 722 and so far, only on the 72.7 satellite channels. My EA dish was installed last March.

I called Dish last week and a tech came out. He saw the dish on top of a second story roof and said he "didn't want to go up there today".

He started diagnosing the cause by everything but the lnbf.
He removed the 5 year old ribbon cable a previous installer used and drilled a hole with my OK.
He swapped out the triplexer behind the receiver and the 004 error still intermittently occurs.
He had a new receiver to swap, but I wouldn't have had time to move my recordings to my EHD because he wanted to take the old one with him. He said to call when it was convenient for me and ask that a replacement be shipped.

So, could a defective receiver be the cause of this error, or is it most likely an lnbf??

Well, yes, but you have two receivers that show the same error, so I think that pretty strongly argues for a bad LNB, or a bad cable between the dual and the twin. If you start swapping components yourself, in order of cheapness and ease, I'd (1) replace the cable between dual and twin, then I'd (2) replace the dual, and finally I would (3) replace the twin.
