I've seached but am not totally clear on exactly which slots E* ended up receving approval for. I think EX-4 and EX-5 will go at 75w and 62w, is that correct? What are the other ones?

They are all here. Look for the Echostar EX satellites for the locations and license details.

There is some double sat name labeling in the licences but the locations and beams are correct ( the original application covered multiple slots and the application slot names and final license slot names do not necessarily agree).

additional channels, presumably, Both DirecTV and Dish (plus others) have slots awarded for use of this newly allocated band.
