Dish Anywhere app on screen pairing not working

I reset my Dish Anywhere app to try to get downloads to work (it's an old version one locked to a Fire tablet). In doing so I have lost the ability to "log in" using the on-screen pairing. This isn't only on the older app - it fails on the most recent app on my cell phone too.

For the older app I try to use it and it gives an error message "Request failed" and I have to use the web based credentials. After logging in I can use it to view programming on the DVR and live programming, but attempting to authorize the device for transfers fails with "User not found". It's an old version of the app...]

For the new app on-screen pairing is not offered as an option. Instead I have to enter my web based credentials.
