Sure can, I have that. How it works is you end up with Top 250 for 10$ more above the DVRA Top 200. Top 250 is basically treated as Premium.

So in my case I have DVRA Top 200, then DishHD+3Prem (50$), for Top 250, HBO, and Starz.
