Help - Need 6 outputs on a Dish 500 DP-Quad

I have the following:
Dish 500 DP Quad
2 - 811
1 - 522 (di-plexing one input/output to broadcast whole house)
1 - 921

How do I hook it all up to the dish and also OTA?
Can you di-plex a separator? For example, use separator to run one line to 921 but also di-plex to run OTA.

Do not have any external switches.

With purchasing the 921, I can have Dish do a free install of the 921. Would I be better off doing that if they will provide the necessary switches, separators, di-plexors?

Have DISH do the free install you need a DP-34 so pay $100 or get it free from DISH.
