How to report broken disc?

Just got my first disc and it arrived broken. Not off to a good start.

I see no way to report the disc broken. In my queue under disc problem, there is nothing to choose from. Am I missing something?
 
Are you here? http://www.blockbusterathome.com/queue/fullQueue If so, you should see a "Report Problem" link to the right of the disk at home, and there should be a pop up menu with an item to report a damaged disk. (I can't check this for sure because I have a store disk right now, and the pop up menu is a little different.)

Thanks. That link worked. I was just going to blockbuster.com. All my account info is there, including my queue and movies at home, but the report problem does not show up under disc problem.

It would be under the "Discs at Home" on the far right is the Report a Problem link. Please PM me your account or phone number if you are still having an issue. Thank you!
