Leasing, is there an authorization form that must be signed?

I am about to lease a home, it is NOT an apartment, it is a house. Does Dish Network require an authorization form to be signed by the owner before they will install? I have the owners permission, i just want to get the form signed early before installation if required since she will be out of state.

Wouldn't hurt to have but I believe that dish no longer requires it. I dont ask for them but if the cust has one for me when I get to a job I'll attach it to a site survey and turn it in for filing.

I would get one to be safe.

Nope!

Your responcible for obtaining permission for the install before the technician arrives.

So you get permission, and the technician is going to assume you got it before they perform the install
