have seen some of these on TNT and espn. thought this was happening for awhile but yesterday is was a local dentist with just 2 offices here in Vegas. phone # with no area code.

how are they doing this ?
 
Basically the local commercials are downloaded or recorded to the hard drive and the broadcast is marked to insert local commercial in certain spots and on certain channels.
Similar to what cable does, but much more difficult since it is on a national level with localized content.

Targeted ads are downloaded to the customer’s DVR, then inserted seamlessly to the most relevant audience in both live and playback viewing

Take a look at this FAQ page for advertisers, it sums up how it works and why Directv invested so much in setting this up. In fact they partnered with Dish in this venture and they market it together in some situations.
