Poor Quality on Locals

I live in the Louisville KY market, and I have two questions.
#1 is: how does dish get the locals from the broadcasters, do they have an antenna in each market and then have it feed back to them with fiber, or are they feeding directly from the broadcasters via fiber back to dish or what?

#2 is: lately I have been noticing poor quality in my locals that I get from dish. One of the local channels has been having a bad problem with ghosting and audio sync issue during last December and they seem to have that working better now, and a few others have got a slight ghosting problem. I contacted dish at there quality issue e-mail address, and did get a reply back that they would look at the problem, and it doesn't look like they have fixed it very good, as there still a problem with ghosting. Is there anything that I could do/call to report the reaming issues?
