Receiver fee breakdown and install question

Does any one have the current receiver fee breakdown? I've searched and am having a hard time finding it. I'm looking for the fee breakdowns for:
1 722
1 722 and 1 211
1 hopper and 2 Joeys

Also. If I have 1 722 currently on a 1000.4, what would I need in the form of splitters and such to add a 211

Just to clarify, Tampa is including account level DVR and MRV fees in his receiver fees summary. The primary receiver on the account is always deemed as included in the cost of your programming package.
1 722- no receiver fee, $6 DVR fee
1 722 and 1 211- $6 DVR fee, $7 fee for the 211
1 Hopper and 2 Joeys- $6 DVR fee, $4 MRV fee, $7 fee for each Joey

When I added a 211k to my system.I called DISH and setup an appointment,installer came out ran RG6 coax from the LNB to the bedroom of my choice,setup the 211k.The cost was $15.(I have the DISH Service Plan).:)

I also believe there would be a 24 month commitment to lease a 211k,no commitment if you buy one.
