Bought my PS3 2 months ago sent back the form along with the UPC bar code and still no movies in the mail. Who do I contact to find out what the delay is?

Did you send in the receipt also, marked up as the form stated?
And BTW, did you buy from Amazon? If so, what did you send in as a receipt- the paper (packing slip) that was inside the box? Or the email confirmation?
