The SW21 is a legacy switch, which means you can't feed two tuners with one cable. The cheapest solution is to cable up the outputs of your duals to TWO SW21's, and run the outputs to the two 612 tuners with two wires.
You might not want to do that, though, since you will be (at minimum) living without your HD locals. You need a 3rd satellite location to get that.

You need to upgrade your dishes and lnb's to receive full HD service.
You need a Dish 1000.4 (about $100) or add a D300 for 61.5 with appropriate switches.
People who upgrade to HD should do it through Dish and they will install the correct hardware and you do not have to suffer through these problems.
