Basic question about the price

I will be living probably soon in the US, and I would like to have some information about the prices of the HD option (because I am not sure ti have understood everything on the official web site) :

Could you tell me, regardless of special offers, what is the price of the HD option (in addition to the monthly channel package) :
- if you are a previous DirecTV customer ?
- for a new customer ?

I pay $9.95/month for the HD package on Directv as a previous customer.

Did you have any additional cost for the installation ?

Could somebody confirm me that to get this option, you have to have a minimum package of $49,99/month ? (I mean the "CHOICE" package and that for example HD option is not possible with the FAMILY package)
