On Dish Network's website, is "Opt out of offers" the same as calling Dish to end my three free month's of HBO and Showtime, (and Sundance, and whatever else is included)?

Yes it's similar to calling. You can click on the "change" option on the right, and change the "status on expiration" to "Auto Remove". This will make it so they automatically drop off when the promotion expires.
