I just experienced a complete deletion of all my recording in the Primetime folder as well as all of my other recordings. My hard drive was about 55% full and I never had any issues like this in the past.

I also had all of my timers deleted and when I tried to back up the timers from my remote I got a error.

The folder with rented movies was left alone and all of my guides so I know this wasn't a factory reset.

I have a the newer HWS that's about 2 years old.

Has anyone had any issues with this before?

Wish that would have cured my loss of programs and I had known of that trick. It said "all programs were lost but timers were saved." Timers were saved in remote only. And yes I lost a near full disk of programs that I was reorganizing--switching from HDD by IMDb rating to alphabetic, so that it does not change every week. After the second time in a week that it lost them all, I asked and received a new HWS. I cannot replace many of those movies. Maybe some day they will come by again.
