Yes to all. Costco, $100 for you and $100 for the other party spread out over 10 months.

Yes you can get both. Get your account set up through the Costco offer and then call the referral hotline after your account is setup and tell them you forgot to add the referral. You just need to call for the referral prior to your installation date.
