Dish 'N it Up Rebate

For my rebate I had to send a copy of my bill indicating the requisite programming, etc. But, having paperless billing I had to download the pdf from Dish's website. That particular bill failed to read every time.

I emailed ceo@echostar.com and the next day they credited my account with the rebate amount.
 
To me, it looks as if I have to wait for the full (paperless) billing to show the upgrade. This even though the Activity page show $199 DIU 921(?)->722 upgrade--that is not a bill. So it will be a month before I can start the process for the $100!

BTW, the same form name (Q1.349_RedemptionForm.pdf) dated 7/7 did not include the upgrade as a rebate. Why not use a new name?

Guess I lose DHP for DHA because I am returning my original but inactive 501--still no RMA, have original box but I think they cannot send return slip without another box. Why? Deactivating my 921--for $10, worth it as OTA tuner. Add a charge and take it off, again and again. Why?
