Do you have to have a phone line plugged in any more?

If you did not receive the loyalty credit then you are not going to get it. No need for the roulette. It only applied to certain accounts depending on the actual price jump.
 
Look on your bill and see if it has a section like I put below. If it is not there, it does not hurt to call Dish and complain about the sudden rate hike you just saw on your bill when they raised the per box fees. Perhaps CSR roulette might be in order to get the credits.

I hate playin CSR roulette, I just wish dish would automatically give everyone the same deal and not play favorites with the squeaky wheel types .
