Issues with D*

Does anybody know where and who to send complains to D* about their
CSR. I have customer that D* claims to be one of their best customer and
yet one of the CSR she spoke to said that she would have to pay $20.00 to
replace a defective old access card. I never heard of this before. Thanks for
your help.
 
They charge for the FedEx shipment of new cards, and in almost every case, if the new card is activated within the 14 day alloted time, the $20 is credited back to the account, thus FREE. This has been going on for a while; company mandated card swaps net the "free on the front end" cards. You can always escalate your calls to supervisors and/or call customer retention, but there is nothing amiss here with this request.
