New house - wiring advice

I am in the process of having a new house built, I'm a couple of weeks away from wiring. Currently I'm on an ancient system that I set up myself 12 years ago, I see things are quite different now. Given the opportunity to specify how I want the house wired, can someone please offer suggestions?

I will have 1 HD/DVR and 4 SD.
 
Determine where the dish should be mounted.
Run RG6 from the location, either one or four to a central/common location in the house.
From there you need to run RG6 to each room requiring a receiver.
If you aren't going to have SWM, then you'll need two lines to those rooms.
This location is where the distribution equipment will be installed.

I have an addition to jdspencer's post. Put in 1 spare cable of all types that you are going to use to each location while the rooms are un finished. This is the cheapest solution as running cables after the rooms are finished will involve more time and effort to get a cable to the room that needs it after the room is finished.
 
While you're at it, you should determine in which room your modem and router are going to be and run some cat6 cable from that room to any other rooms you want to have a hardwired network connection (any room with a computer, xbox, hd-dvr, etc).
