He does have a point, why are the Premiums not all in HD? After all if Dish wants near $20 (19) for HBO, they should be able to offer it all in HD.

He didn't mention HBO at all. In fact all the HBO channels except HBO2W are in HD. We pay a pretty good premium for HBO. Maybe we'd have the Showtimes, all in HD, if we were willing to pay the extra to get them...

I specified HBO since it was the most expensive. That said, all the other premiums are pretty expensive and should be worthy of a 100% HD treatment.
