I called a CSR when I got my second statement and no rebate was applied. Lady stated the rebate form was received, but they take 6 - 8 weeks to process it. So, it's more like the third, possibly fourth statement that the rebate will kick in.

I also have the same problem using Vonage and I've tried all sorts of things like using a DSL filter and *99 prefix. The strange thing is that I can use the modem in my computer to connect to my ISP and surf the web over Vonage, yet I can't connect to Dish?
